A Study to Assess The effectiveness of Almond Oil Massage on promotion of Breast Milk Secretion among LSCS Mothers in selected hospital Bhilai (C.G.)

ABSTRACT:
This study was conducted with the objective to. assess the breast milk secretion before application of almond oil massage among LSCS mothers. to administer the almond oil on breast among LSCS mothers to assess the breast milk secretion after the administration of the almond oil massage among LSCS mothers to determine the effectiveness of almond oil massage on promotion of breast milk secretion among LSCS mothers. to find out the association between effectiveness of almond oil massage on promotion of breast milk secretion with selected socio demographic variables. The study design was an pre-experimental, one group pre-test post-test research design. 40 samples of LSCS mothes who fulfil the inclusive criteria were selected based on non-probability purposive sampling, after getting informed consent from the study participant, the almond oil massage was administered to LSCS mothers at Shri shankaracharya Medical College and Research centre, Bhilai C.G. The study findings revels that the pre-test score regarding breast milk secretion is 12(30%) sample were in inadequate, 24(60%) were moderate and 4(10%) adequate. In post-test score majority 33(82.5%) were in adequate, 7(17.5%) sample were in moderate and 0 (0%) were in inadequate. The ‘t’ value 13.35 (df.39) was greater than table value (2.02) at 0.05 level of significance.

INTRODUCTION:
“Know your Rights and Demand your Rights”
Breast milk is produced by a human Female and fed to infants by breast feeding. The breast milk is very healthy and full of nutrition, it provides the primary source of nutrition for newborns before they are able to digest more diverse food.1 The breast milk also contains balanced nutrients that are require for brain development, growth and a healthy immune systems that act against viruses, bacteria and parasitessince an infant’s immune system is not fully developed until age of 2 years, human milk provides a distinct advantages over formula.2
Breast milk has important ingredients that are not found in any infant formula, to build baby’s immune system.3 Breast milk changes from feed to feed to suit each baby’s unique needs, making the perfect food to promote healthy growth and development. Breast milk is more easily digested than infant formula. Breastfed Babies are rarely constipated and are less likely to get diarrhoea. Breast milk has no waste products and leaves no carbon foot print.4
Breast feed babies are low risk of gastrointestinal illness, allergies, asthma, Diabetes, obesity, some childhood cancer, Respiratory tract infection, Urinary Tract infection, SIDS (cot death) The uniqueness and precious nature of breast milk is enhanced by the fact that it is asset given by nature and has no price.5 The vital components for the infants in tropical countries are breast feeding and avoidance of infections as we know so many advantages of breast feeding it is easily digestable, protecting against infections. It isreadily available, it contains.6
lactoferin which hinders growth of E-coli. In studies shown that 80% of infants growing healthy who receiving breast feeds.7
NEED FOR THE STUDY:
Promotion of breast feeding is one among the goals of World Health Organization. Many mothers have been followed to achieve this goals such as establishment of children-friendly hospital and conducting breast feeding education for the mothers. Despite all the conducted activities a high number of mothers feed their infants with formulas due to their inadequacy of breast milk.Almond oil massage is a type of herbal traditional medicine which increases mother’s milk by increasing the secretion of prolactin hormone.8
Almond oil massage over breast with the goal of improving the health and promotion relaxation and clears the ducts in turn can promote lactation in breastfeeding mothers.And its ability to induce a relaxed state in the body have been found to assist postpartum mothers with milk production.9
OBJECTIVES:
1. To assess the breast milk secretion before application of almond oil massage among LSCS mothers.
1. To administer the almond oil on breast among LSCS mothers.
2. To assess the breast milk secretion after the administration of the almond oil massage among LSCS mothers
3. To determine the effectiveness of almond oil massage on promotion of breast milk secretion among LSCS mothers.
4. To find out the association between effectiveness of almond oil massage onpromotion of breast milk secretion with selected socio demographic variables.
H0: There will be no significant effectiveness of almond oil massage in promotion of breast milk secretion among LSCS mother.
H1: There will be significant effectiveness of almond oil massage in promotion of breastmilk secretion among LSCS mother.
H2: There will be significant association between pre-test score of breast milk secretion among LSCS mothers with their selected socio-demographic variables.
CONCEPTUAL FRAMEWORK:
Model attempts to represent reality with a minimum use of words. Visual or symbolic representation of theory or conceptual frame work often help to express abstract, ideas in a more readily understandable or precise form than the original conceptualization. The conceptual framework of this study is based upon Sister Callista Roy’s Adaptation model which has five components – person, goal of nursing, health, environment and nursing activities.

Population:
Target population:
In present study the target population included were LSCS mothers.
Accessible Population:
In present study the Accessible population included were LSCS mothers at shri Shankaracharya Medical College and Research Centre C.G.
Setting of The Study:
The setting for the present study was postnatal ward of Shri Shankaracharya Medical College and Research Centre C.G.
Sample:
The sample of the study comprised of 40 LSCS mothers of Shri Shankaracharya Medical College and Research Centre C.G.
Sampling Technique:
Non probability Purposive sampling technique was used to select 40 samples.
Sample Size:
Total 4 0 postnatal LSCS mothers were taken for this study.
Variables:
According to Polit and Hungler, variable is an attribute of a person or an object that varies, that it takes an different values. Two type of variable are identified in the study. They are dependent variable and independent variables.
Dependent variable:
Dependent variable was– promotion of Breast milk screation
Independent variable:
Independent variable was Almond oil massage over the breast.
Sampling Criteria:
Inclusion criteria:
· LSCS mothers who had present at the time of data collection.
· LSCS Mothers who are willing to participate in this study.
Exclusion criteria:
· Postnatal mothers who had normal delivery.
· Postnatal LSCS mothers who had any postnatal complications.

The self structured questionnaires were organised in two sections:

The breast milk adequacy checklist prepared for assess the promotion of breast milk secretion.
The score categorized as:
· Adequate (1-6)
· Moderate (7-13)
· Inadequate (14-20)

Method of Data Collection:
The data collection was done for 4 weeks period. After obtaining informed and willing from the study participants the study was conducted at the shri Shankaracharya Medical College and Research Centre C.G. total 40 samples were collected.
Data Analysis and Interpretation:
Descriptive statistics such as frequency, percentage, mean and standard deviation was used to assess the. Effectiveness of almond oil massage on promotion of breast milk secretion among LSCS mothers.

Table shows that the Pre-test and Post-test breast milk secretion score was 358 and 630 respectively. The total mean and mean score percentage was 8.95(44.75%) and 15.75(78.75%) respectively. The standard deviation of pre-test breast milk secretion score was3.02 and post-test breast milk secretion score was 2.26.
Criteria wise analysis of pre-test and post-test Breast milk secretion among LSCS mother by frequency and percentage.
Table and Fig: Shows that in pre-test score regarding breast milk secretion is 12(30%)sample were in inadequate,24(60%) were moderate and 4(10%) adequate. In post test score majority 33(82.5%) were inadequate,7(17.5%) sample were in moderate and 0(0%) were inadequate.

Table represent that there was highly significant difference between pre-test and post-test breast milk secretion score as calculated ‘t’value 13.35 (df.39) was greater than table value (2.02) at 0.05 level of significance.
Hence the Alomond oil massage was effective on promotion of breast milk secreation among LSCS mothers:
Major Findings of Study:
The data was collected, analysed and interpreted in term of objectives. Descriptive and inferential statistics were utilized for the data analysis. The level of significance set for testing the hypothesis was, 0.05 using paired ‘t’ test. The major findings of the study were presented under the following
Description of subjects according to socio-demographic variables in frequency and percentage:
1. Most of the LSCS mothers 22(55%) belonged to above 35 years of age.
2. Maximum Numbers 18(45%) LSCS mother were studied up to secondary education.
3. Majority of LSCS mothers 13(32.5%) were in Government job.
4. Maximum number 21(52.5%) LSCS mothers were belonged with urban area.
5. Majority of LSCS mother 21(52.5%) were followed Non-vegetarian diet.
6. Majority of LSCS mothers 30(75%) belonged were hindu religion.
7. Majority of LSCS mothers 22(55%) were belonged to joint family.
8. Maximum numbers 21(52.5%) LSCS mother had Multipara.
9. Majority of LSCS mother 19(47.5%) were belong to 4th day of post natal period.
10. Most of the LSCS mother 15(37.5%) had frequency of breast feeding is usually 5-6 times.
Analysis by using mean, mean % and standard deviation:
Result shows that the Pre-test and Post-test breast milk secretion score was 358 and 630 respectively. The total mean and mean score percentage was 8.95(44.75%) and 15.75(78.75%) respectively. The standard deviation of pre-test breast milk secretion score was 3.02 and post-test breast milk secretion score was 2.26.

Chi Square Analysis for Association Between The Pre Test score Of Breast Milk Secretion with Selected Socio-Demographic Variables.
· There is significant association between pre-test breast milk secretion and age of subject as the calculated chi square value is 14.99 (df-6) is more than the table value of chi square 12.59 at 0.05 level of significance.
· There is no significant association between pre-test breast milk secretion and qualification of subject as the calculated chi square value is 4.77(df-8) is more than the table value of chi square 15.51 at 0.05 level of significance.
· There Is significant association between pre-test breast milk secretion and occupation of subject as the calculated chi square value is 17.13(df-6) is more than the table value of chi square 12.59 at 0.05 level of significance.
· There is no significant association between pre-test breast milk secretion and area of residence of subject as the calculated chi square value is 4.48 (df-2) is more than the table value of chi square 5.99 at 0.05 level of significance.
· There is no significant association between pre-test breast milk secretion and food habit of subject as the calculated chi square value is 6.02 (df-4) is more than the table value of chi square 9.49 at 0.05 level of significance.
· There is no significant association between pre-test breast milk secretion and religion of subject as the calculated chi square value is 5.66(df-8) is more than the table value of chi square 15.51 at 0.05 level of significance.
· There is no significant association between pre-test breast milk secretion and types of family of subject as the calculated chi square value is 1.44(df-4) is less than the table value of chi square 9.49 at 0.05 level of significance.
· There is no significant association between pre-test breast milk secretion and order of pregnancy of subject as the calculated chi square value is 4.07(df-2) is less than the table value of chi square 5.99 at 0.05 level of significance.
· There is no significant association between pre- test breast milk secretion and post-natal period of subject as the calculated chi square value is 4.98(df-4) is less than the table value of chi square 9.49 at 0.05 level of significance.
· There is significant association between pre- test breast milk secretion and frequency of breast feeding of subject as the calculated chi square value is 15.17(df-6) is less than the table value of chi square 12.59 at 0.05 level of significance.
Hence hypothesis H2 is accepted.
CONCLUSION:
The breast milk is very healthy and full of nutrition; it provides the primary source of nutrition for newborns before they are able to digest more diverse food. The breast milk also contains balanced nutrients that are required for brain development, growth and a healthy immune system that act against viruses, bacteria and parasites since an infant’s immune system is not fully developed until age of 2 years, human milk provides a distinct advantages over formula. Breast Feeding should commence as soon as possible after giving birth and every 1 to 3 hours per 24 hours (8-12 times/24 hours) Babies should be breast fed exclusively for the 6 months.
Almonds oil cashews are accepted to lift bosom milk creation. Studies demonstrated devouring almond oil during lactation it advance milk emissions as aides in combination of nutrient B additionally helps in emulsification of globules. As almond oil contains unsaturated fats, proteins, and calcium and liolenic acids. It likewise contains protein 6g, iron 1.2g, calcium 70 mg, dietary fiber 3.3g, potassium 206 mg, nutrient E 7.3mg, magnesium 78 mg and phosphorous 134 mg. The conclusion of the study shows that almond oil massage was found to be an effective alternative therapy in increasing breast milk screation.
